By completing this form, you hereby agree to hold ReNue, its doctors and affiliates harmless from any hacking or any other unauthorized use of your personal information by outside parties. By submitting your photo, you grant ReNue permission to use your photo for all purposes, including marketing and promotion. You also confirm that you have all rights necessary to grant this permission, including from all subjects in the photo and the person who took the photo.

All offers are open to Canadian residents age 18 or older whom our doctors deem to be candidates for treatment. The Grand Prize Winner will be selected by the doctor(s) and staff of ReNue. Photo entries will be posted to our Facebook page and the photo with the most "Likes" will be determined the Runner-Up Winner. Should the Grand Prize Winner not be a good candidate for the selected Grand Prize treatment, a medical spa treatment of equal or lesser value may be substituted at the discretion of ReNue.
